Tax Identification Number	The tax identification number of the business.
Physical Business Address	The address of the business.
Ownership Type	The ownership type of the business.
Date of Establishment	The date on which the business was established.
Phone Number	The phone number of the business.
Email Address	The email address of the business.
Website	The website of the business.
Do you process over $250K annually?	The processing volume of the business.

Social Security Number	The owner's social security number.

SS-4 form - To verify the TIN associated with the business.
Articles from SOS - To verify active articles have been filed with the state.
Address Verification such as Driver's License, Utility Bill, and Business Lease - To verify the business or home address.
